One week Hair transplantation fellowship

I, Dr.________________, Age______ Years, Address: Hereby agree to join one week fellowship in hair transplantation, from date____________.

I have been informed that this one week course is an observation course and will consist of observation of hair transplant surgery in the theatre. It will include practical training in dissection of hairs and daily discussion of different aspects of hair transplantation.

I have also read and full understand the following conditions and limitations, as stipulated by Venkat Charmalaya:

Since this is a cosmetic surgery and the patients are all private patients, student will not be allowed to handle the patient independently at any time. Patients privacy , patients rights, and patient’s interest will be kept upper most

How much exposure and opportunity for the trainee depends totally on the requirements of a given case and the decision and judgement of the senior doctor. Any procedure in which the trainee has to participate, prior permission will be sought from the patient. Patient permission, patient’s safety interests and medico legal aspects will govern the involvement of trainee in all cases. The decision of the senior staff will be final in all circumstances. It is advised that the student read the available material on the internet thoroughly. The teaching will be didactic and will consist of questions and answers

We generally have surgery almost daily; however this can not always be predicted and guaranteed. So it is possible that there may not be any surgeries on certain days.

Discretion of the senior doctor will be final in all matters.
